Senior Fitness Readiness Pathway Singapore

A PAR-Q-style, HPB/GAQ-aligned readiness pathway for older adults, caregivers and families before starting strength training, balance work, walking programmes or home personal training.

This checklist is for exercise readiness and fitness planning only. It is not a medical diagnosis, medical clearance, physiotherapy assessment or emergency assessment.

If you have chest pain, fainting, sudden weakness, severe breathlessness, new neurological symptoms, uncontrolled blood pressure, a recent serious fall or unexplained pain, please seek medical advice before exercising.

How This Screening Works

1. Safety Readiness Checks for symptoms, medical concerns and activity restrictions before exercise.
2. Activity Baseline Estimates current weekly aerobic, strength and balance activity.
3. Senior Function Highlights fall risk, mobility confidence, caregiver support and supervision needs.
Step 1: Pre-Exercise Safety Readiness

Answer honestly. If unsure, choose “Yes” and seek advice from a doctor, physiotherapist or qualified healthcare professional before increasing exercise intensity.

In the past 6 months, have you been treated for a heart condition or stroke, or felt chest pain, chest pressure or unusual discomfort during daily activity or exercise? 过去6个月内,是否曾因心脏疾病或中风接受治疗,或在日常活动/运动时出现胸痛、胸闷或不适?
Have you been diagnosed with high blood pressure, or is your resting blood pressure around 160/90 mmHg or higher? 是否曾被诊断为高血压,或静息血压约为160/90 mmHg或更高?
Do you feel dizzy, lightheaded or unsteady during physical activity? 运动或体力活动时是否会头晕、眼花或明显不稳?

Have you fainted, lost consciousness or had a recent concussion? 是否曾晕倒、失去意识,或近期有脑震荡?
Do you currently have pain, swelling, injury, arthritis flare-up, back pain or joint pain that affects movement? 目前是否有疼痛、肿胀、受伤、关节炎复发、背痛或关节痛,影响活动能力?
Has a doctor, physiotherapist or healthcare provider advised you to avoid or modify certain exercises? 医生、物理治疗师或医护人员是否曾建议您避免或调整某些运动?
Do you have a medical or physical condition that may affect exercise, such as diabetes, cancer, osteoporosis, asthma, kidney disease, neurological condition or spinal condition? 是否有可能影响运动的疾病或身体状况,例如糖尿病、癌症、骨质疏松、哮喘、肾病、神经系统或脊椎问题?

Step 2: Current Physical Activity

Stop Exercise and Seek Help If These Occur

Stop the session if there is chest pain, fainting, severe breathlessness, sudden weakness, unusual confusion, new severe headache, severe dizziness, loss of balance, or pain that feels unsafe.

How to Read Your Result

Green Pathway Start low, progress gradually, monitor response and build consistency.
Amber Pathway Use modified, supervised and lower-intensity exercise with caregiver awareness where needed.
Red Pathway Seek medical review or professional clearance before starting or increasing exercise.

A high readiness score does not override chest pain, fainting, severe breathlessness, uncontrolled symptoms, recent serious illness, or medical advice to avoid exercise.
